Concussion Policy

When a student is medically diagnosed with a concussion, he/she must return a physician-signed Concussion Return to Play form to the school nurse in order to resume PE/athletic activities.

Grades K-8 use the STE Concussion Return to Play Form
Grades 9-12 use the TAPPS Concussion Return to Play Form
